Howl-o-ween Pet Costume Contest

OFFICIAL CONTEST RULES AND REGULATIONS: FVAH CLIENTS – IMPORTANT.

Please read the following Official Contest Rules and Regulations before submitting anything. By participating in the Contest, you, as the  submitter, agree to these Official Rules and Regulations and represent that you satisfy all of the eligibility requirements listed below. HOW TO ENTER. Entrants should FIRST read all of the Official Contest Rules and Regulations before submitting anything. To  enter, submissions must contain all of the following information: one (1) photo of your pet(s) dressed up in a Halloween costume(s) (Note: the image size  MUST be less than 2.5 mb), the name of the pet(s) in the image, and the entrant’s/pet owner’s first name, last name, email address, and phone number. For  each entry, also be sure to include a brief description of your pet(s) and the costume(s). Submissions should be emailed to connectinghearts@forevervets.com with all the information provided above. You should receive a response within 24-48 hours with an entry number, along with further instructions.

SUBMISSION GUIDELINES AND RESTRICTIONS. Only one (1) submission allowed per household. The Submission must not contain material that violates,  misappropriates, or infringes upon any law or regulation or the rights of any third party, including any copyright, trademark, or any rights of publicity or  privacy, or any other intellectual property or proprietary rights. The Submission must not disparage any person or entity or contain material that is  inappropriate, indecent, obscene, hateful, torturous, and/or defamatory. The Submission must not contain material that promotes bigotry, racism, hatred or  harm against any group or individual or promotes discrimination based on race, gender, religion, nationality, disability, sexual orientation or age. Any entry  that does not follow and adhere to these guidelines and restrictions (as determined by Forever Vets Animal Hospital in its sole and absolute discretion) will  be discarded and will not be eligible for entry in this Contest. Forever Vets Animal Hospital does not guarantee that all Submitted photos will be published.

USE OF WINNER’S NAME, LIKENESS, ETC. By sending in your Submission photo you: (a) grant Forever Vets Animal Hospital all rights necessary to  display your Submission on Forever Vets Animal Hospital’s website and Forever Vets Animal Hospital’s Facebook page; and (b) hereby waive any so-called  moral (e.g.. , creative rights) in your Submission. Except where prohibited by law, entry into the Contest constitutes permission to use winner’s name,  Facebook name, and/or pet’s name(s) in all media now known or later devised throughout the universe in perpetuity for all purposes Forever Vets Animal  Hospital deems appropriate – including, without limitation, for promotional and publicity purposes – without further permission or compensation. As a  condition of being awarded any Prize, except where prohibited by law, winner may (in Forever Vets Animal Hospital’s sole discretion) be required to  execute a written consent, confirming Forever Vets Animal Hospital’s right to use such winner’s name, Facebook name, and/or pet’s name(s) without  further permission or compensation.

SUBMISSION PERIOD. Entries may be submitted at the start of the Submission Period starting at 12:00 am Eastern Standard Time on October 1st, 2025. The deadline for submitting entries is 11:59 pm Eastern Standard Time on October 15th, 2025. All entries must be received during the Submission Period to be eligible to win any prize. Any entries submitted before or after this time-frame will not be accepted.

VOTING PERIOD. The Voting Period will start at 12:00 pm Eastern Standard Time on October 16th, 2025. Qualifying entry photos will be posted to the  Forever Vets Animal Hospital Facebook brand page photo album titled, “ Halloween Pet Costume Contest 2025” at the start of the Voting Period. The Voting Period will end at 12:00 am Eastern Standard Time on October 29th, 2025. Please note: no “likes” will be counted as a vote after the voting portion ends.

WINNER SELECTION AND NOTIFICATION. The three (3) entry photos (as posted by Forever Vets Animal Hospital) with the highest number of Facebook  photo “likes” by the end of the Voting Period, will be deemed the Contest winners. The winners will be announced to the Forever Vets Animal Hospital  Facebook page on October 30th, 2025 (in the morning) and by email from connectinghearts@forevervets.com. Winners will be notified within three (3) days of the Contest winners announcement posted at the conclusion of the  Contest. If a Selected Entrant cannot be contacted or fails to respond to any message within forty-eight hours (48 hours) of the first attempt of contact by  Forever Vets Animal Hospital, he/she will be considered to have forfeited his/her Prize and will be disqualified, and another Entrant may be selected, in the  sole discretion of Forever Vets Animal Hospital, from the remaining eligible entries until contact is made with a Selected Entrant, or there are no more  eligible Entries, whichever comes first. In the event that an alternate Selected Entrant is required to be selected, the same above-noted process will apply.  Forever Vets Animal Hospital has the right to cancel the contest promotion at any time.

CONTEST PRIZES. Three (3) Winner(s) will be selected to win one (1) of the following three (3) prizes: the first place Winner (the entrant(s) photo with the  most Facebook “likes”) will receive $150.00 credit to his/her Forever Vets Animal Hospital account, the second place Winner (the entrant(s) photo with the  second highest number of Facebook “likes”) will receive $100.00 credit to his/her Forever Vets Animal Hospital account, and the third place Winner (the  entrant(s) photo with the third highest number of Facebook “likes”) will receive $50.00 credit to his/her Forever Vets Animal Hospital account.

TIES. In the unlikely event of a tie (i.e. 2 or more entries obtain the same amount of Facebook photo “likes” on their submission photo as posted by Forever Vets Animal Hospital) each relevant entrant will receive the original prize credit amount listed and shall not be divided equally amongst winners.

NO FACEBOOK ENDORSEMENT. This Contest is in no way sponsored, endorsed or administered by, or associated with Facebook, Inc. Any information  you provide in connection with the Contest is to Forever Vets Animal Hospital and not to Facebook. You understand that by using and interacting  with www.facebook.com, you are subject to the terms, conditions, and policies that govern the use of www.facebook.com. You should therefore review the  applicable terms and policies for www.facebook.com, including privacy and data gathering practices, before using or interacting with Facebook.
